Quality program for high-risk COPD, tested with primary care teams

This study looks at how well a quality-improvement program (called CONQUEST) helps people with higher-risk chronic lung disease (COPD) when delivered through primary care teams. It compares the program given right away versus usual care for a while, then the program later.

Who can take part

  • You are a primary care team (not an individual patient) that will be assigned to an approach.
  • Your team can be randomly assigned as one group without too much “mixing” between team members or clinicians.
  • Your practice has enough COPD patients who meet “modifiable high-risk” criteria to make results reliable.
  • Your team agrees to be randomly assigned to either start CONQUEST right away or use usual care first, then get CONQUEST later.
  • Your team cannot be in the middle of, or planning, changes to electronic health record (EHR) software or practice ownership during the study period.
  • Your team cannot already be running a COPD quality-improvement project (or active research) that would make it hard to run CONQUEST.
