  • You are between 18 and 75 years old.
  • You have been diagnosed with CKM stage 0, 1, or 2 (early stages of heart-kidney-metabolic syndrome).
  • You can use a smartphone and a wearable activity tracker.
  • You are willing to sign a consent form and attend follow-up visits.
  • You do not have advanced CKM (stage 3 or 4), a serious illness needing hospitalization, or active cancer.
