Nutrition education groups for adults with type 2 diabetes

Who can take part

  • You must be an adult (18 or older) with a past diagnosis of type 2 diabetes
  • Your most recent A1C blood test must be above your target but not above 12%
  • You should not have severe nerve damage related to diabetes (severe neuropathy)
  • Your kidneys must be working enough (kidney filtration rate 30 or higher)
  • You must be able to join education activities and not have major cognitive/neurological/psychiatric limits
  • You must not be pregnant or breastfeeding and must avoid recent heart attack symptoms (acute coronary syndrome in last 60 days)
