- You are 18 years old or older
- You have a first-time heart-cancer (cardio-oncology) consultation for your case
- Your consultation is for one of these: breast cancer, Hodgkin/non-Hodgkin lymphoma, leukemia, multiple myeloma, AL amyloidosis, or immune checkpoint inhibitor heart evaluation
- You have not already been evaluated and followed by these study investigators for cardio-oncology before
- You are able and willing to consent (agree) to take part
