Who can take part

  • You are age 18 or older and live in a Native American tribal community in the southeastern U.S.
  • You have high blood pressure (recent readings) or you currently take medicine for it.
  • You can pass a short mini-memory/thinking screening test.
  • You can walk on your own without a walker or needing someone’s physical help.
  • You do not have severe vision or touch problems that would stop you from using a phone or sensors.
