  • You are scheduled to receive, or previously received, the Equioxe Proximal Humerus Fracture Plate system
  • You agree to return for pre- and post-surgery study visits
  • You agree to be followed for up to 10 years after your surgery
  • You can review, understand, and sign the study consent form
  • You are able and willing to follow the surgeon’s post-surgery care instructions
  • You are not pregnant and are able to comply with study follow-up rules
