- You were hospitalized for a recent heart event like a heart attack or a heart procedure to improve blood flow
- You live within 30 miles of the University of Vermont Medical Center (UVMMC)
- You are older than 18
- You currently smoke cigarettes (any cigarette use in the week before admission, and usually more than 5 cigarettes per day over the last 3 months)
