Who can take part

  • You’re 18 or older and live in Ontario with OHIP coverage.
  • Your doctor has diagnosed you with type 1 diabetes (not another type).
  • You currently use an insulin pump or take insulin as multiple daily injections.
  • Your recent blood sugar control is higher than a certain level (HbA1c 7.5% or above, or a similar estimate from your CGM/FGM in the last 4 months).
  • You can reliably use the internet and have a phone, computer, or tablet with a video camera.
  • You had a recent visit (within the last 6 months) with a certified diabetes educator at a participating clinic.
  • You can speak and read English, are not pregnant, and are not on dialysis.
