  • You have type 2 diabetes
  • You are 18 years or older
  • You have Medicaid (or both Medicaid and Medicare)
  • You use insulin to manage your diabetes
  • You own and regularly use a smartphone (Android or iPhone) for at least 6 months
  • You can read text messages on your phone, and can use apps beyond just calling and texting
