- Your baby is 0 to 90 days old
- Your care team expects your baby will need vancomycin for at least 48 hours
- Your baby was born with a corrected (adjusted) gestational age of at least 25 weeks
- Your baby weighs at least 500 grams
- Your baby has no known allergy to vancomycin or similar antibiotics
- Your baby has not received vancomycin in the past 72 hours
