- Had acute kidney injury (sudden kidney function loss) during a hospital stay
- 60–90 days have passed since your kidney function was at its worst
- For some parts: need a baseline kidney function test from the past 12 months
- Able to understand and agree to participate in the study
- Not currently on dialysis (artificial kidney cleaning) or had a kidney transplant
