- Be between 18 and 65 years old
- Live in Harlem and come from low-income housing or get primary care in Harlem
- Have depression screening results showing at least mild risk (PHQ-4 total score 3 or higher)
- Not screen positive for severe mental health concerns like psychosis, mania, substance misuse, or very high suicide risk
