  • You are an adult (at least 18 years old) when you enroll
  • You have a diagnosis of metabolic syndrome or related conditions, or your records show BMI 30 or higher
  • You must have access to your medical record (EHR) with at least one encounter in the last 3 years
  • You can complete consent paperwork (written informed consent)
  • If you join the survey part, you must be willing to finish the questionnaires within the expected timeline
