  • Your baby was born very early: under 26 weeks, or small for gestational age born at 26 to 31 weeks and 6 days
  • Your baby weighs at least 450 grams at birth
  • The pregnancy was a single baby or twin (not triplets or more)
  • Your baby’s sex can be male or female
