Who can take part
- Be age 65 or older
- Have diabetes or high blood pressure plus at least one other long-term health condition
- Have had 2+ clinic visits in the last 12 months and plan to keep seeing the same clinician for the next 6 months
- Have access to internet and speak English or Spanish
- Be able to understand and consent (no major cognitive issues on the study screener)
- Have not used your own visit recordings for personal use in the last 6 months
