- Baby must weigh at least 5.5 pounds (2500 grams) at birth
- Baby must be full-term, born between 37 and 42 weeks of pregnancy

- Baby gets care at our pediatric primary care clinic
- Baby is from a racial/ethnic minority group (non-white, Hispanic, or Latinx) or from a low-income background (household income at or below 138% of the federal poverty level or on Medicaid)
