Who can take part

  • You are a teenager or young adult with type 1 diabetes, age 16 or older
  • You are in the process of moving from pediatric to adult diabetes care at Royal North Shore Hospital or Mona Vale Community Health Centre, or you made this move between 2020 and June 2023
  • You are a family member or caregiver of such a patient
  • You are a healthcare staff member on the diabetes care team for these patients
