Who can take part

  • You are 18 years old or older
  • You had a first-ever stroke diagnosed in the hospital within the last 6 months
  • You can walk on your own (using a walker or cane is okay if needed)
  • Before your stroke, you were independent with daily activities like dressing, bathing, and eating
  • You live at home (not in a rehab facility or nursing home)
  • You are not currently in a physical rehab program that lasts 3 months or longer
