  • You were 19 years old or older when you entered the ICU.
  • You needed a breathing machine (ventilator) while in the ICU.
  • You were in the ICU for at least 24 hours.
  • You were not already on a breathing machine at home, in assisted living, or in a long-term care facility.
  • You are not currently in prison or jail.
