  • You are between 18 and 80 years old.
  • You have had a past heart or blood vessel event, such as a heart attack, stroke, or procedure to open blocked arteries.
  • You visit a cardiology or medical clinic at one of the listed hospitals in Hong Kong.
  • You can understand English or Traditional Chinese.
  • You do not have dementia, a serious mental health condition, or a physical disability that would prevent participation.
