Who can take part

  • You are pregnant and it is no later than 20 weeks along.
  • You are considered high-risk (for example, if you are overweight/obese before or early in pregnancy, or have had diabetes or high blood pressure before).
  • You are receiving care through Community Health Choice in Houston, Texas.
  • You speak English or Spanish.
