  • You are pregnant and receiving care at a Harris Health clinic
  • You have Medicaid or are eligible for it
  • You are 14 weeks pregnant or less at your first prenatal visit and enrollment
  • You are at high risk for gestational diabetes (BMI 30 or more plus a second risk factor like age over 35, certain ethnicities, history of diabetes in a prior pregnancy, or other listed conditions)
  • You live within the study's delivery area
