- You have been diagnosed with type 2 diabetes.
- You have not been using a continuous glucose monitor (CGM) — a device that checks your blood sugar all the time.
- You do not use insulin for your diabetes.
- You speak English, Spanish, or Arabic.
- Your most recent A1c blood test (within the last 90 days) was between 7.5% and 12.0%.
- You have a cellphone that can send and receive text messages and count your steps.
