- You are an adult (18+) who is Hispanic/Latino and a registered Scripps Health patient
- You have type 2 diabetes
- Your recent HbA1c (a blood sugar marker) is at least 8% within the last 60 days
- You can come to clinic visits and you are not too ill to participate
- You can read and understand the program materials and join by phone/text in Spanish or English
