  • You have high blood pressure that is not well controlled (140/90 or higher).
  • You are a patient at Neighborhood HealthSource in Minneapolis and have a main doctor there.
  • You own a smartphone and can use the internet at least once a week.
  • You have an email address and basic internet skills.
