- Your child is a patient at one of the four Legacy Community Health school-based clinics in Galena Park, Texas.
- Your child's BMI is at or above the 85th percentile (overweight or obese).
- You live within 10 miles of the Houston Food Bank.

- You can read and write in English or Spanish to fill out surveys.
