  • Your child is seen at Cherokee Health Systems (CHS) in Cherokee Mills
  • Your child is between 4 and 10 years old
  • Your child’s body mass index (BMI) is above the 85th percent for their age
  • An adult caregiver (over 18) lives in the home and agrees to take part
  • The adult caregiver can read and speak English
