Who can take part

  • You are age 18 or older
  • You join the Hospital at Home program after the emergency room visit or as an inpatient who can be cared for at home
  • A clinician decides your diagnosis fits one of the program’s hospital-level conditions (not chosen by researchers)
  • You live within about 30 minutes’ drive of the hospital
  • You do not need long-term nursing facility care and you are not currently living in one
  • You can follow the program’s home-care plan set by the hospital team
