  • You are 61 years old or older
  • You have been diagnosed with frailty (a Clinical Frailty Score higher than 4)
  • You are having a planned (elective) surgery
  • You speak English fluently or comfortably
  • You do not live in a nursing home or assisted living before surgery
  • You are able to use video calls or phone for follow-up care
