- You identify as Black, African American, or Afro-Caribbean
- You are between 50 and 92 years old
- You have had pain in your hips, knees, lower back, or ankles for at least 6 months
- You feel aching, stiffness, or swelling in a major leg joint or lower back on most days of the week
- You have a health insurance plan that works in Florida (like Medicare, Medicaid, or private insurance)
- You can read and understand English at about a 6th-grade level
