- You are enrolled in the CareSource Medicaid plan.
- You go to a primary care clinic that uses the Epic electronic health record system and is within 60 miles of Case Western Reserve University in Cleveland.
- You are willing to sign a consent form and stay in the area for the next year.
- You do not have a serious medical condition (like dementia or severe mental illness) that would prevent you from participating.
