Who can take part
- You are a primary care doctor, nurse practitioner, physician assistant, nurse, or social worker at a UNC Health clinic that sees at least 60 patients with dementia each year.
- You care for older adults (age 65+) at your clinic.
- For the training part, you must work at a clinic chosen to receive the new tool.
- If you are a person with dementia, you must be 65 or older and have a confirmed diagnosis of Alzheimer's disease or a related dementia.
- You must have been seen by your primary care team in the last 18 months.
- You have a high chance (50% or more) of passing away in the next 5 years, for the health use analysis part.
