Who can take part
- The resident must have a confirmed dementia diagnosis (any stage).
- The resident must have moved into the assisted living facility within the last month.
- You must be the resident's legal representative who speaks English.
- The resident cannot already be receiving hospice care.
- The assisted living facility must be licensed in Florida, have at least 40 beds, and serve memory care to at least 35% of residents.
