- You were seen in a Froedtert & Medical College of Wisconsin emergency department.
- You are 18 years or older.
- You were diagnosed with a mild traumatic brain injury (also known as a concussion) by a doctor.
- You speak English or Spanish.
- You are not in protective custody or on a psychiatric hold.
- You are your own legal decision-maker (no activated healthcare power of attorney).
