Who can take part
- You are 70 years old or older
- You are eligible for open-heart surgery
- You have two or more of these health risks: stroke, memory or thinking problems, COPD (long-term lung disease), obesity, weak heart pumping, kidney failure, trouble walking, previous heart surgery, or complex surgery needed
- You can read and understand Dutch
