  • You are 80 years old or older.
  • You are active and healthy for your age (scored as 1, 2, or 3 on a frailty scale).
  • You have a primary care doctor who works with an advanced practice nurse.
  • You have health insurance (affiliated to a Health Care Plan).
  • You can understand French and travel to your doctor's office.
  • You must give signed consent to join the study.
