  • You have been diagnosed with heart failure.
  • You have been on the same medications for at least 3 months as recommended by your doctor.
  • You are 18 years or older.
  • You are able to walk and come to the clinic.
  • You have not had a heart attack, a stent, or heart surgery in the past 3 months.
