Who can take part
- You are 55 years or older.
- You have a heart condition that qualified you for cardiac rehab, such as a heart attack, stent, bypass surgery, heart failure, or valve replacement.
- You attended more than half of your scheduled cardiac rehab sessions and are about to finish the program.
- You are not currently enrolled in an extended, out-of-pocket cardiac rehab program.
- You do not have serious memory problems or difficulty understanding English or Spanish.
- You do not have unstable heart rhythms, uncontrolled high blood pressure, or other active heart or health issues that make exercise unsafe.
