- You are 18 years or older.
- You are currently in the hospital at Massachusetts General Hospital after a second heart attack (or a likely one).
- Your primary care doctor is part of the Mass General Brigham network and you've seen them in the last 2 years.
- You are expected to be able to walk on your own when you leave the hospital (no walker or oxygen needed).
- You are willing and able to give your consent to join the study.
