  • You are an employee, spouse, or domestic partner of a participating company.
  • You are 21 years or older.
  • You have been diagnosed with metabolic syndrome (3 or more of: high waist size, high triglycerides, low good cholesterol, high blood pressure, or high blood sugar).
  • You do not have a history of certain mental health conditions like eating disorders, major depression, or severe anxiety.
