- You received NobelReplace, NobelActive, or NobelParallel implants with TiUnite or TiUltra surfaces starting in October 2021 or later.
- You were 18 years or older when your implant was placed.
- You came back for a check-up at least one year after surgery.
- Your implant stability was checked with RFA (a device that measures how well the implant is held in place) at least three different times.
