- You are 40 years old or older.
- You have been diagnosed with COPD by a doctor.
- You live in a rural or small-town area (not a big city or suburbs).
- You have some shortness of breath with everyday activities (mMRC score of 1 or more).
- You can walk (even if with some difficulty) and you do not have memory or thinking problems that would make it hard to follow instructions.
