This trial tests whether a new way of identifying and treating hidden blood vessel disease in the brain can lower the risk of stroke and dementia. It is for people over 65 who have had certain findings on an old brain scan but have never had a stroke or memory problems.

Who can take part
- You are 65 years or older
- You have never had a stroke, mini-stroke, dementia, Parkinson's, multiple sclerosis, or a cancerous brain tumor
- You had a CT or MRI brain scan in the last 5 years that showed signs of small vessel disease or an old silent stroke
- You are able to communicate by email, letter, or phone and can make your own decisions about participating
