- You are 65 years of age or older.
- You are scheduled for or have had a same-day (day) surgery like an appendectomy, hernia repair, a biopsy, or gallbladder removal.
- You can hear and see well enough to talk on the phone, or a family member can help.

- You have no more than two chronic conditions, and those must be diabetes and/or high blood pressure.
