  • You or your child has a chronic condition like high blood pressure, diabetes, high cholesterol, asthma, or depression.
  • You or your child receives care at a Family Medicine clinic that is part of this study.
  • If you are an adult (18 or older), you must have at least one of these conditions: high blood pressure, high cholesterol, diabetes, or depression.
