- You have type 2 diabetes.
- You have seen a primary care doctor in this health system in the last 2 years.
- Your kidney lab tests show moderate to severe kidney disease (based on urine protein or blood filtration rate).
- You are not already on all recommended kidney-protective medicines (like SGLT2 inhibitors or ACE/ARB or finerenone).
- You do not have type 1 diabetes, a prior kidney transplant, or a specific genetic kidney disease (ADPKD).
- You are not pregnant or planning pregnancy within the next year.
