Who can take part

  • You are a patient at Michigan Medicine and have electronic health records from 2020 onward.
  • You are 18 or older.
  • You own and use an Android or iPhone smartphone daily.
  • You understand English for surveys and notifications.
  • You completed cardiac rehab in the last 30 days or will finish within 30 days for coronary artery disease, bypass surgery, valve repair/replacement, or heart attack without surgery.
