- Be a patient at Michigan Medicine with a heart failure diagnosis
- Be 18 years or older
- Have class 2 or 3 heart failure symptoms (e.g., some limitation in activity)
- Be cleared by your doctor for safe home exercise
- Own a smartphone (iPhone or Android) and be comfortable using it
- Be fluent in spoken and written English
- Not have been hospitalized for heart failure in the last 6 weeks
