  • You are between 18 and 90 years old
  • You were just in the hospital for a heart failure flare-up (your heart's squeezing ability is still normal, but it doesn't relax well)
  • You are going straight home after discharge (not to a nursing facility or rehab)
  • You are not receiving homecare or hospice services
  • You are willing to take part in follow-up phone calls
  • You do not have dementia or a condition that makes it hard to give your own permission
