Who can take part
- Admitted to the hospital for heart failure within the last 3 days
- A blood test showing high NT-proBNP (at least 1,000 pg/mL) during this stay
- Blood pressure and heart rate within normal ranges (systolic at least 100 mmHg, heart rate at least 60 bpm) in the last day
- Currently on a low dose of common heart failure medications (ACE inhibitor, ARB, ARNi, beta-blocker, or MRA)
- No serious kidney disease, recent heart transplant, active cancer, or conditions that prevent using a smartphone
